Spousal Eligibility Review
Over the past several months, the Benefits Fund has been conducting an eligibility review to ensure that all spouses currently enrolled in the Fund meet our eligibility requirements. The deadline for submission of the required documents was May 24, 2022.
If your documents are not received at the Fund office by June 15, 2022, Benefits Fund coverage for your spouse, including medical, prescription drug, dental and vision care, will be terminated effective July 1, 2022.
Because the official deadline has passed, you must submit your documents directly to the Benefits Fund. You may do so by choosing one of the submission options below. Do NOT submit your documents to Cotiviti as instructed on the paperwork you previously received in the mail.
Mobile Upload
This is the easiest way to provide your signed paperwork to the Fund. Use the button below to access our secure upload site. Once there, enter your first and last name and email address in the correct fields. For the company field, please type an X. Then, use the “Browse files” link to use your camera to take a photo and upload it in one step. Once you have uploaded your document, the upload box will show that the document was successfully uploaded.
